the barnacle hands has two modes to switch with right click one rappeling and pull the pull is the grapple and rappeling to get the enemy near you and eat him for grapple you need to push left click and don't stop pushing the click if it gets on a wall the tongue then stop the pushing

you get it before this level. A scientist tells you that they have it and then you get it in a room with redlights.

how do you get the barnacle grapple? just yank a barnacle off the wall and start using its tentacle to stick to stuff? XD
